Overview

Bendamustine + alemtuzumab is a combination regimen pairing the small-molecule alkylating agent bendamustine with the anti-CD52 monoclonal antibody alemtuzumab, investigated primarily in relapsed/refractory chronic lymphocytic leukemia. Bendamustine causes DNA crosslinking and apoptosis; alemtuzumab targets CD52 on B and T lymphocytes to induce antibody-dependent cellular cytotoxicity and complement-mediated lysis. Phase I/II studies reported that this combination (often termed BenCam) achieved high overall response rates in heavily pretreated CLL, with bendamustine given intravenously on days 1–2 and alemtuzumab administered subcutaneously on days 1–3 per cycle, aiming to control both nodal and circulating disease while managing myelosuppression and infectious risks.
