Overview

Bendamustine + melphalan is a combination chemotherapy regimen used primarily as a high-dose conditioning treatment before autologous stem cell transplantation (ASCT) in patients with multiple myeloma. Bendamustine is an alkylating agent with both nitrogen mustard and purine analog properties, while melphalan is a classic alkylating agent. Both drugs act by cross-linking DNA strands, thereby inhibiting DNA replication and transcription, leading to cell death. The combination aims to improve the depth of remission compared to melphalan alone, as shown in randomized phase II trials where the addition of bendamustine increased complete remission rates without significant additional toxicity[1][3][4]. This regimen is sometimes referred to as "BenMel" in clinical studies.
