Benralizumab + omalizumab is a combination of two monoclonal antibody biologics used off-label as dual biologic therapy in patients with severe, refractory type 2 inflammatory diseases such as allergic bronchopulmonary aspergillosis (ABPA), chronic spontaneous urticaria (CSU), and severe asthma when monotherapy with either agent alone is insufficient. Benralizumab targets the interleukin 5 receptor alpha subunit (IL5Rα) on eosinophils and basophils, leading to their depletion via antibody-dependent cell-mediated cytotoxicity. Omalizumab binds to circulating IgE, preventing its interaction with high-affinity IgE receptors on mast cells and basophils, thereby reducing allergic inflammation. The rationale for combining these agents is to simultaneously target distinct but complementary pathways in type 2 inflammation—IgE-mediated hypersensitivity and eosinophilic inflammation—providing improved disease control in select patients unresponsive to single-agent biologic therapies[1][2][5].
