Benzathine penicillin + etanercept is a combination therapy being investigated by Nanfang Hospital, Southern Medical University, for the treatment of spondyloarthritis and ankylosing spondylitis. This regimen combines benzathine penicillin G, a long-acting beta-lactam antibiotic that inhibits bacterial cell wall synthesis by binding to penicillin-binding proteins, with etanercept, a biologic TNF receptor fusion protein that neutralizes tumor necrosis factor (TNF). The rationale for this combination in inflammatory rheumatic diseases is to evaluate whether the addition of an antibiotic to standard biologic therapy can improve clinical outcomes, disease activity scores, and physical function, potentially by addressing underlying infectious triggers or modulating the microbiome. It is currently being evaluated in a Phase 4 clinical trial.
