Benzocaine + phenylpropanolamine is a combination drug consisting of two active ingredients with distinct mechanisms of action. Benzocaine is a topical local anesthetic that works by blocking nerve signals in the body, providing temporary relief from pain and itching by deadening nerve endings at the site of application[6]. Phenylpropanolamine (PPA) is a sympathomimetic agent structurally related to amphetamines and pseudoephedrine; it acts primarily as an alpha-adrenergic receptor agonist, leading to vasoconstriction and appetite suppression[5]. This combination has been used in non-prescription formulations for weight loss, with benzocaine intended to reduce taste sensation (and thus food intake), while PPA suppresses appetite centrally. However, clinical studies have shown that combining benzocaine with phenylpropanolamine does not enhance weight loss compared to PPA alone but does increase adverse effects[3][4]. Due to safety concerns—particularly an increased risk of hemorrhagic stroke associated with PPA—products containing phenylpropanolamine have been withdrawn from many markets including the United States[5][7].
