Betamethasone + hyaluronidase is a combination drug consisting of betamethasone, a potent synthetic glucocorticoid corticosteroid with anti-inflammatory and immunosuppressive properties, and hyaluronidase, an enzyme that hydrolyzes hyaluronic acid in connective tissue to increase tissue permeability. This combination is used topically or via injection for conditions where enhanced penetration of the steroid is desired. Betamethasone acts by binding to the glucocorticoid receptor and modulating gene expression to suppress inflammation. Hyaluronidase increases the diffusion of betamethasone through tissues by breaking down extracellular matrix components, thereby facilitating better absorption and distribution of the corticosteroid. The combination has been studied primarily for topical treatment of phimosis in children and as intralesional therapy for oral submucous fibrosis[1][2][3][4]. Clinical studies indicate that while both agents are effective together or alone compared to placebo, adding hyaluronidase does not significantly improve efficacy over betamethasone alone but may enhance local drug penetration[1][3].
