This is a combination therapy consisting of bevacizumab (a monoclonal antibody targeting vascular endothelial growth factor A), capecitabine (an oral prodrug of 5-fluorouracil, a pyrimidine analog antimetabolite), and radiotherapy. Bevacizumab inhibits angiogenesis by binding to VEGF-A and preventing its interaction with receptors on endothelial cells, thereby reducing tumor blood supply. Capecitabine is converted to 5-FU in the body and interferes with DNA synthesis in rapidly dividing cancer cells. Radiotherapy uses ionizing radiation to damage DNA in cancer cells directly or indirectly through free radical formation. This combination has been studied primarily as neoadjuvant treatment for locally advanced rectal cancer[1][2]. The regimen aims to enhance tumor cytotoxicity by combining anti-angiogenic effects, chemotherapy-induced cytotoxicity, and direct DNA damage from radiation.
