This is a four-drug combination regimen consisting of bevacizumab, carboplatin, gefitinib, and pemetrexed. It is being investigated as a first-line treatment for patients with advanced non-squamous non-small cell lung cancer (NSCLC) harboring epidermal growth factor receptor (EGFR) mutations. Bevacizumab is a monoclonal antibody that inhibits vascular endothelial growth factor A (VEGF-A), thereby blocking angiogenesis and tumor blood supply. Carboplatin is a platinum-based chemotherapeutic agent that causes DNA crosslinking and apoptosis in rapidly dividing cells. Gefitinib is an EGFR tyrosine kinase inhibitor that blocks signaling pathways involved in cell proliferation and survival in tumors with activating EGFR mutations. Pemetrexed is an antifolate antimetabolite that inhibits multiple enzymes involved in folate metabolism and DNA synthesis, leading to suppression of tumor cell replication. In recent phase Ib clinical studies, this combination has shown high objective response rates (ORR 95%) and disease control rates (DCR 100%), with manageable toxicity profiles for patients with EGFR-mutated advanced non-squamous NSCLC[6][9].
