This is a combination therapy consisting of three agents—bevacizumab, dexamethasone, and verteporfin—used primarily for the treatment of neovascular (wet) age-related macular degeneration (AMD). Each component targets a different pathological process in AMD: - **Bevacizumab** is a monoclonal antibody that inhibits vascular endothelial growth factor A (VEGF-A), reducing neovascularization and vascular leakage. - **Dexamethasone** is a synthetic glucocorticoid corticosteroid with potent anti-inflammatory effects, helping to control inflammation associated with choroidal neovascularization. - **Verteporfin** is a photosensitizer used in photodynamic therapy (PDT); upon activation by light, it generates reactive oxygen species that selectively damage abnormal blood vessels. The rationale for this triple therapy is to combine complementary mechanisms—anti-angiogenic, anti-inflammatory, and vessel-targeting—to improve visual outcomes while potentially reducing the frequency of anti-VEGF injections. This regimen has been studied in patients who are either treatment-naïve or have failed monotherapy with VEGF inhibitors[1][3][7][8].
