Overview

Bevacizumab + evofosfamide is an investigational combination therapy for cancer. Bevacizumab is a monoclonal antibody that inhibits vascular endothelial growth factor A (VEGF-A), thereby blocking angiogenesis and reducing tumor blood supply. Evofosfamide (also known as TH-302 or Evo) is a hypoxia-activated prodrug that releases the DNA crosslinking agent bromo-isophosphoramide mustard under low oxygen conditions typical of solid tumors. This combination has been studied primarily in patients with recurrent glioblastoma who are refractory to bevacizumab alone. The rationale for the combination is that resistance to antiangiogenic therapy like bevacizumab can increase tumor hypoxia, making tumors more susceptible to activation of evofosfamide and its cytotoxic effects[1][3][6]. Clinical trials have shown modest activity in this setting but have not led to regulatory approval[1][4][7].
