A-FOLFOX + A-FOLFIRI is a combination chemotherapy regimen used as a comparator arm in clinical trials for metastatic colorectal cancer, specifically for right-sided tumors. The regimen involves the administration of the anti-VEGF monoclonal antibody bevacizumab (Avastin) in conjunction with one of two standard doublet chemotherapy backbones: FOLFOX (infusional fluorouracil, leucovorin, and oxaliplatin) or FOLFIRI (infusional fluorouracil, leucovorin, and irinotecan). Bevacizumab functions by binding to vascular endothelial growth factor A (VEGF-A), thereby inhibiting tumor angiogenesis. The chemotherapy components act as cytotoxic agents: fluorouracil inhibits thymidylate synthase, leucovorin enhances fluorouracil activity, oxaliplatin causes DNA cross-linking, and irinotecan inhibits topoisomerase I. This treatment strategy is evaluated as a first-line therapy in patients with unresectable metastatic disease.
