Bevacizumab + fotemustine is a combination therapy consisting of bevacizumab, a monoclonal antibody targeting vascular endothelial growth factor A (VEGF-A), and fotemustine, an alkylating agent of the nitrosourea class. Bevacizumab inhibits angiogenesis by binding to VEGF-A, thereby preventing its interaction with VEGF receptors on endothelial cells and reducing tumor blood vessel formation. Fotemustine exerts cytotoxic effects by alkylating DNA and causing cross-linking, leading to cell death. This combination has been investigated primarily for the treatment of recurrent glioblastoma and high-grade gliomas, particularly in patients who have relapsed after standard therapies such as radiotherapy and temozolomide[1][2][5]. Clinical studies suggest that the regimen may offer disease control and clinical benefit in this setting; however, superiority over monotherapy with either agent has not been conclusively demonstrated[1][3][5].
