Overview

Bevacizumab + utidelone is a combination therapy consisting of bevacizumab, a monoclonal antibody targeting vascular endothelial growth factor A (VEGF-A), and utidelone, a novel epothilone-class microtubule inhibitor. Bevacizumab inhibits angiogenesis by binding to VEGF-A, thereby preventing the growth of new blood vessels that supply tumors. Utidelone stabilizes microtubules and disrupts mitosis in cancer cells; it is structurally distinct from other microtubule inhibitors like paclitaxel and can evade common resistance mechanisms such as P-glycoprotein-mediated efflux. This combination has shown promising efficacy and manageable safety in clinical trials for HER2-negative metastatic breast cancer with brain metastases, as well as non-small cell lung cancer with brain metastases[1][3][4][6]. Utidelone’s ability to cross the blood-brain barrier makes this regimen particularly relevant for central nervous system involvement[5][6].
