MATINS-02 is an investigational combination immunotherapy consisting of bexmarilimab, a first-in-class humanized monoclonal antibody targeting the immunosuppressive scavenger receptor Clever-1 (Stabilin-1), and an anti-PD-1 monoclonal antibody. Developed by Faron Pharmaceuticals, this combination is designed to overcome primary resistance to PD-1 blockade in immunologically "cold" solid tumors, such as gastric, gallbladder, cholangiocarcinoma, and ER+ breast cancers. By blocking Clever-1 on tumor-associated macrophages (TAMs), bexmarilimab reprograms these cells from an immunosuppressive M2-like phenotype to an immunostimulatory M1-like phenotype, upregulating interferon-gamma production and priming the tumor microenvironment to restore sensitivity to PD-1 checkpoint inhibition.
