Bexmarilimab + doxorubicin is an investigational combination therapy being developed by Faron Pharmaceuticals, primarily for the treatment of soft tissue sarcomas. Bexmarilimab (FP-1305) is a humanized IgG4 monoclonal antibody that targets Clever-1 (Common Lymphatic and Endothelial Vessel Receptor-1), a scavenger receptor expressed on immunosuppressive M2 macrophages. By blocking Clever-1, bexmarilimab facilitates the conversion of these pro-tumoral macrophages into a pro-inflammatory M1 phenotype, thereby activating the immune system against the tumor. Doxorubicin is a well-established cytotoxic chemotherapy that works by intercalating into DNA and inhibiting topoisomerase II, leading to DNA damage and cell death. This combination, evaluated in the Phase 1 BEXAR trial, aims to synergize the direct cytotoxic effects of chemotherapy with immune-modulating macrophage reprogramming to improve outcomes in patients with advanced solid tumors.
