BH3120 + pembrolizumab is an investigational combination therapy comprised of **BH3120**, a bivalent bispecific antibody targeting PD-L1 and 4-1BB, and **pembrolizumab**, an established PD-1 antagonist checkpoint inhibitor. **BH3120** uses Hanmi’s Pentambody platform to simultaneously block the **PD-L1** pathway and agonize **4-1BB**, selectively stimulating anti-tumor T cell activity within the tumor microenvironment while minimizing systemic immune activation[1][2][5]. When used together, BH3120 and pembrolizumab act synergistically: pembrolizumab releases the PD-1 checkpoint on T cells, allowing enhanced 4-1BB clustering and activation by BH3120, thereby promoting cytokine release (IL-2, IFN-γ, TNF-α), lymphocyte expansion, and reinvigoration of exhausted T cells for more robust anti-tumor responses. The combination targets multiple immune checkpoint and co-stimulatory axes (PD-1/PD-L1, PD-1/PD-L2, PD-L1/CD80, 4-1BB). Preclinical and early clinical studies show dose-dependent synergistic activity, rapid tumor regression, and a favorable safety profile[1][2][5]. The lead indication is solid tumors.
