**BHV-1100 + CIML NK cells** is a combination therapy for multiple myeloma (MM) consisting of BHV-1100, a fully synthetic bispecific antibody-recruiting molecule (ARM) that binds CD38 on MM cells and endogenous immunoglobulin (Ig) to recruit natural killer (NK) cells for antibody-dependent cellular cytotoxicity (ADCC), paired with autologous cytokine-induced memory-like (CIML) NK cells primed with IL-12, IL-15, and IL-18 and coated with BHV-1100 ("ARMored") to enhance tumor killing without fratricide. Developed by Biohaven, this regimen is administered as a single-dose infusion post-melphalan conditioning and prior to autologous stem cell transplant (ASCT) in MRD-positive patients in first remission, often with IVIG and low-dose IL-2; preclinical data show superior ex vivo killing of MM cells (e.g., 90.8% vs. 81% at 24h), increased IFNγ and CD107a, with in vivo persistence peaking at day 28.[1][3][5]
