BI-505 + melphalan is an investigational combination therapy being studied for the prevention or delay of relapse in patients with multiple myeloma undergoing autologous stem cell transplantation (ASCT). **BI-505** is a human monoclonal antibody that specifically binds to intercellular adhesion molecule 1 (**ICAM-1**, also known as **CD54**) on myeloma cells. It acts through two main mechanisms: it induces direct cell death in myeloma cells and recruits macrophages to attack these cells. **Melphalan** is a bifunctional alkylating agent that covalently binds DNA, causing DNA damage and resulting in cell death. High-dose melphalan is standard-of-care conditioning chemotherapy prior to ASCT for multiple myeloma. The combination is intended to leverage melphalan’s cytotoxic effect with BI-505’s immune-mediated anti-myeloma activity, with the goal of improving progression-free survival.
