**BI 765049 + ezabenlimab** is a combination of two investigational cancer immunotherapies currently in clinical trials. - **BI 765049** is a novel, IgG-like bispecific T-cell engager antibody designed to bind simultaneously to B7-H6 on tumor cells and CD3 on T cells, redirecting T cells to kill B7-H6–positive cancer cells through cytolytic synapse formation and local immune activation. - **Ezabenlimab** (also known as BI 754091) is a monoclonal antibody that inhibits the programmed cell death protein 1 (PD-1) checkpoint, reducing tumor immune evasion by blocking the PD-1/PD-L1 axis. The combination aims to enhance immune-mediated tumor killing: BI 765049 directs T cells to attack B7-H6–positive tumor cells, while ezabenlimab helps sustain T cell activity by preventing PD-1–mediated inhibition, which can be induced as a resistance mechanism during B7-H6-directed T-cell engagement. Both agents are delivered by intravenous infusion. The combination is being studied for advanced solid tumors expressing B7-H6, including colorectal, gastric, pancreatic, hepatocellular, head and neck, and non-small cell lung cancer, especially in patients who have progressed on or are ineligible for standard therapies[3][5][7][8].
