This is a combination of six broad-spectrum antibiotics—biapenem, meropenem, piperacillin, tazobactam, tigecycline, and levofloxacin. Each component targets bacterial pathogens through distinct mechanisms: - **Biapenem** and **meropenem** are carbapenems that inhibit bacterial cell wall synthesis by binding to penicillin-binding proteins (PBPs), leading to cell lysis and death[6][8][9]. - **Piperacillin** is an extended-spectrum penicillin antibiotic that also inhibits PBPs; it is often combined with **tazobactam**, a beta-lactamase inhibitor that protects piperacillin from enzymatic degradation by beta-lactamases. - **Tigecycline** is a glycylcycline antibiotic that binds to the 30S ribosomal subunit, inhibiting protein synthesis. - **Levofloxacin** is a fluoroquinolone antibiotic that inhibits DNA gyrase and topoisomerase IV, enzymes essential for bacterial DNA replication. This combination would provide extremely broad antibacterial coverage against Gram-positive and Gram-negative bacteria—including many multidrug-resistant organisms—and would be considered only in highly resistant or life-threatening infections where no single agent or standard combination suffices. Such combinations are not standard clinical practice but may be used in salvage therapy for severe infections due to extensively drug-resistant bacteria.
