A combination therapy consisting of bicalutamide, a first-generation nonsteroidal androgen receptor antagonist, and androgen deprivation therapy (ADT), which suppresses the production of androgens (typically via medical castration using GnRH agonists or antagonists). This regimen is primarily used in the treatment of advanced or metastatic hormone-sensitive prostate cancer. Bicalutamide blocks the androgen receptor, while ADT lowers circulating testosterone levels, together achieving more complete androgen blockade. This combination (often called \"combined androgen blockade\") has been standard care for metastatic prostate cancer, although newer, more potent androgen receptor pathway inhibitors are increasingly favored in recent guidelines for improved outcomes.
