Bicalutamide + finasteride is a combination therapy being investigated by the Sidney Kimmel Comprehensive Cancer Center at Johns Hopkins for the treatment of prostate cancer, specifically in patients with MRI-detectable nodules undergoing active surveillance. Bicalutamide is a non-steroidal antiandrogen that acts as a competitive antagonist at the androgen receptor, blocking the action of testosterone and dihydrotestosterone (DHT). Finasteride is a 5-alpha reductase inhibitor that prevents the conversion of testosterone to the more potent DHT. By combining these two agents, the therapy aims to achieve more complete androgen blockade within the prostate, potentially reducing the progression of low-risk nodules and improving the negative re-biopsy rate.
