Overview

The combination of BIO-11006 + carboplatin + pemetrexed is a therapeutic regimen that has been studied in clinical trials for non-small cell lung cancer (NSCLC). This combination includes an investigational peptide (BIO-11006) alongside two established chemotherapy agents. ## Drug Information BIO-11006 is an anti-MARCKS (myristoylated alanine-rich C Kinase Substrate) peptide that is administered via inhalation. It has shown potential antimetastatic effects in lung cancer models[2][3]. The peptide appears to cause cytoskeletal rearrangement in cancer cells, favoring adherence to substrate rather than movement, which may explain its anti-metastatic properties[2]. Carboplatin and pemetrexed are established chemotherapy drugs often used together as a standard of care (SOC) treatment for non-small cell lung cancer[7][8][10]. Pemetrexed (Alimta) is administered at 500 mg/m² while carboplatin is dosed according to AUC 5 or 6 using Calvert's formula[7][8][10]. ## Clinical Development A Phase 2 randomized, controlled, multicenter study evaluated the safety and efficacy of BIO-11006 in Stage IIIB NSCLC patients who were not candidates for immunotherapy[7]. These patients also received pemetrexed and carboplatin as standard of care[7]. In July 2019, Biomarck Pharmaceuticals announced that statistical significance (p=0.02) was achieved in improvement of Overall Response Rate (ORR) in the BIO-11006/SOC group compared to standard of care alone at 3 months in their Phase 2 study in NSCLC[7]. ## Mechanism of Action The combination works through multiple mechanisms: 1. BIO-11006 inhibits MARCKS protein function, which appears to prevent cancer cell migration and metastasis[2][9]. 2. Pemetrexed and carboplatin are chemotherapy agents that destroy rapidly dividing cells, including cancer cells[8]. They work by interfering with DNA replication and cell division. ## Safety Profile The combination therapy appears to have a manageable safety profile. When pemetrexed and carboplatin are used together, common toxicities include myelosuppression, liver and renal toxicity[7][10]. However, the addition of BIO-11006 via inhalation appears to be well-tolerated[7][9]. BIO-11006 has been studied in multiple conditions including ARDS, COPD, and NSCLC, with a "well-established safety profile across multiple placebo-controlled studies" involving over 300 subjects[9]. ## Developer The combination therapy is being developed by Biomarck Pharmaceuticals, Ltd., a biopharmaceutical company focused on developing anti-MARCKS technology for the treatment of various conditions including cancer[7][9].
