Transfused Biotin RBCs (BioRBCs) are an investigational diagnostic and research tool used to quantify red blood cell (RBC) survival and volume in clinical settings, particularly in neonates. Developed by Dr. John A. Widness at the University of Iowa, the technique involves labeling RBCs—either autologous neonatal cells or allogeneic adult donor cells—ex vivo with biotin (Vitamin B7) at specific, discrete densities. Once transfused, these labeled cells can be tracked and quantified in the recipient's circulation using flow cytometry. This method allows for the simultaneous study of multiple RBC populations (e.g., fresh vs. stored blood) without the use of radioactive isotopes. It is primarily used as a research intervention to study neonatal anemia and optimize transfusion practices in very low birth weight (VLBW) infants.
