Overview

Biphozyl and Phoxilium are electrolyte replacement solutions used as replacement fluids during continuous renal replacement therapy (CRRT), specifically continuous veno-venous hemofiltration (CVVH) with regional citrate anticoagulation (RCA). Developed and manufactured by Vantive Health (formerly Baxter), these solutions are indicated for critically ill patients with acute kidney injury (AKI) to maintain acid-base balance and electrolyte homeostasis. Biphozyl is formulated with a lower bicarbonate concentration (22 mmol/L) compared to Phoxilium (30 mmol/L) to mitigate the risk of metabolic alkalosis, a common complication when using citrate as an anticoagulant. Both solutions contain essential electrolytes, including phosphate, to prevent depletion during the filtration process. The Medical University Innsbruck conducted the BiPhox-Trial, a Phase II crossover study, to compare the efficacy of these two fluids in stabilizing pH and bicarbonate levels.
