Overview

This is a combination of four beta-adrenergic blocking agents (beta-blockers): bisoprolol, carvedilol, metoprolol succinate, and nebivolol. All four are used individually in the management of cardiovascular diseases such as heart failure with reduced ejection fraction (HFrEF) and hypertension. - Bisoprolol and metoprolol succinate are second-generation cardioselective beta-blockers that primarily block β1-adrenergic receptors on cardiomyocytes, reducing heart rate and myocardial contractility[2][3][6]. - Carvedilol is a third-generation non-selective beta-blocker with additional alpha1-adrenoreceptor antagonism, providing vasodilatory effects[1][3]. - Nebivolol is also a third-generation selective β1-blocker with vasodilatory properties mediated by nitric oxide release[3][4]. Each agent has demonstrated efficacy in reducing mortality and hospitalizations in patients with HFrEF when used individually[2][6]. There is no clinical evidence or guideline supporting the use of all four agents together as a single combination therapy; typically only one beta-blocker from this group would be prescribed at any given time due to overlapping mechanisms and risk of excessive bradycardia or hypotension.
