Overview

**Bivalirudin + clopidogrel** is a combination of two pharmaceutical agents frequently used as an antithrombotic regimen, especially in the context of percutaneous coronary intervention (PCI) for patients with acute coronary syndrome (ACS), including ST-elevation myocardial infarction (STEMI). **Bivalirudin** is a synthetic, specific, and reversible *direct thrombin inhibitor* that blocks thrombin-mediated conversion of fibrinogen to fibrin and both circulating and clot-bound thrombin. **Clopidogrel** is a prodrug antiplatelet agent that irreversibly inhibits the *P2Y<sub>12</sub> platelet ADP receptor*, thereby preventing ADP-mediated activation of the glycoprotein GPIIb/IIIa complex, and thus reducing platelet aggregation. Administered together, this combination provides substantial inhibition of both the coagulation and platelet aggregation pathways, aiming to reduce the risk of arterial thrombosis during and after PCI procedures[1][3][4][5][6].
