This combination therapy consists of bivalirudin, a direct thrombin inhibitor, and eptifibatide, a glycoprotein IIb/IIIa receptor antagonist. It is used as an anticoagulant and antiplatelet regimen during percutaneous coronary intervention (PCI) in patients with coronary artery disease. Bivalirudin works by binding directly to thrombin to prevent clot formation, while eptifibatide inhibits platelet aggregation by blocking the binding of fibrinogen to the GP IIb/IIIa receptor. The combination was specifically studied by LifeBridge Health in the CLEAR PLATELETS-2 trial to evaluate its effects on periprocedural myocardial infarction, platelet reactivity, and clot strength compared to bivalirudin monotherapy in patients also receiving high-dose clopidogrel.
