BL-8040 + cytarabine is a combination of a high-affinity CXCR4 antagonist (BL-8040, also known as motixafortide) and cytarabine, used investigationally to treat acute myelogenous leukemia (AML), especially relapsed/refractory cases. BL-8040 acts by blocking the CXC chemokine receptor 4 (CXCR4), which mediates retention and survival of AML blasts in the bone marrow and contributes to resistance to chemotherapy. In clinical studies, BL-8040 mobilizes leukemia blasts from the bone marrow into the peripheral blood, increasing their sensitivity to cytarabine-based chemotherapy and interrupting the protective tumor microenvironment[1][2][3][5]. Cytarabine is a nucleoside analog antimetabolite that inhibits DNA synthesis in proliferating cells. This combination has shown improved composite response rates and extended overall survival compared to cytarabine alone in phase 2a clinical trials for AML[1][2][3][5].
