BL-M07D1 + pertuzumab is an investigational combination therapy for HER2-positive cancers, composed of BL-M07D1 (international nonproprietary name: trastuzumab brengitecan), an antibody-drug conjugate targeting HER2, and pertuzumab, a monoclonal antibody that binds a different epitope of HER2. BL-M07D1 consists of a humanized trastuzumab backbone, a cathepsin B-cleavable linker, and Ed-04, a camptothecin-derived topoisomerase I inhibitor, delivering cytotoxic payload to HER2-expressing cells and inducing apoptosis via S-phase cell cycle arrest. Pertuzumab inhibits HER2 dimerization, further blocking HER2-driven signaling. The combination is being developed primarily for locally advanced or metastatic HER2-positive breast cancer and is also studied in other HER2-expressing solid tumors. Clinical trials highlight its efficacy and manageable safety profile, with common adverse events being low-grade anemia, neutropenia, and keratitis.
