BL-M14D1 + atezolizumab is a combination therapeutic regimen consisting of BL-M14D1, an antibody-drug conjugate (ADC) targeting Delta-like ligand 3 (DLL3), and atezolizumab, a monoclonal antibody that acts as a programmed death-ligand 1 (PD-L1) inhibitor. BL-M14D1, developed by SystImmune, is designed to selectively deliver a cytotoxic payload to neuroendocrine tumor cells, such as those in small cell lung cancer (SCLC), which frequently overexpress DLL3. Atezolizumab (Tecentriq) is an immune checkpoint inhibitor that prevents the interaction of PD-L1 with its receptors, thereby restoring and enhancing the anti-tumor T-cell response. This combination is currently being evaluated in Phase 3 clinical trials as a first-line treatment for patients with extensive-stage small cell lung cancer (ES-SCLC), aiming to leverage the direct cytotoxic effects of the ADC alongside the immune-potentiating effects of the checkpoint inhibitor.
