This combination therapy regimen consists of the bispecific T-cell engager (BiTE) blinatumomab and the immune checkpoint inhibitors nivolumab and ipilimumab. It is being investigated in a Phase 1 clinical trial (NCT02879695) sponsored by the National Cancer Institute (NCI) for the treatment of relapsed or refractory CD19+ precursor B-cell acute lymphoblastic leukemia (B-ALL) and mixed phenotype acute leukemia (MPAL). Blinatumomab works by linking CD19-positive B-cells with CD3-positive T-cells, inducing direct cytotoxic activity against the leukemia cells. Nivolumab and ipilimumab are monoclonal antibodies that block the PD-1 and CTLA-4 receptors, respectively, to prevent T-cell exhaustion and enhance the anti-tumor immune response. The combination aims to improve outcomes in patients with poor-risk, relapsed, or refractory disease by combining targeted T-cell recruitment with checkpoint blockade.
