BMS + Bone Marrow Aspirate Concentrate is a combination therapy consisting of a surgical procedure and an autologous biological adjunct, developed for the treatment of small primary and non-primary osteochondral lesions of the talus (OCDs). The treatment involves arthroscopic bone marrow stimulation (BMS), such as microfracture, which debrides the lesion and disrupts subchondral blood vessels to initiate a healing response. This is supplemented by the implantation of autologous bone marrow aspirate concentrate (BMAC) harvested from the patient's iliac crest. BMAC contains a concentrated population of mesenchymal stem cells (MSCs), hematopoietic stem cells, and various growth factors intended to enhance the quality of the subchondral plate and promote superior cartilage repair compared to BMS alone. This intervention is currently being evaluated by the Academisch Medisch Centrum - Universiteit van Amsterdam (AMC-UvA) in the OUTBACK clinical trial.
