BN-Brachyury + bintrafusp alfa is a combination immunotherapy approach that pairs a therapeutic cancer vaccine with a bifunctional fusion protein. BN-Brachyury is a prime-boost vaccine consisting of MVA-BN-brachyury-TRICOM (prime) and FPV-Brachyury (boost) designed to induce immune responses against the transcription factor brachyury, which is overexpressed in many solid tumors and plays a role in epithelial-to-mesenchymal transition. Bintrafusp alfa is a bifunctional fusion protein composed of the extracellular domain of TGF-βRII (functioning as a TGF-β "trap") fused to a human IgG1 monoclonal antibody blocking PD-L1. This combination aims to enhance anti-tumor immunity through multiple mechanisms: generating brachyury-specific T cell responses while simultaneously blocking immunosuppressive pathways mediated by TGF-β and PD-L1.
