Bone allograft + bone marrow concentrate is a combination therapy used primarily in orthopedic and reconstructive surgery to enhance bone healing and regeneration. The therapy involves the use of an allogeneic (donor-derived) bone graft as a structural scaffold, combined with autologous (patient-derived) concentrated bone marrow aspirate, which is rich in mesenchymal stem cells (MSCs), osteoprogenitor cells, growth factors, and cytokines. The MSCs from the concentrated bone marrow can differentiate into osteoblasts and other cell types involved in tissue repair, while the allograft provides an osteoconductive matrix for new tissue formation. This combination aims to provide both the cellular elements necessary for regeneration and a physical framework for new tissue growth. It is most commonly used in cases of nonunion fractures, spinal fusion procedures, large bony defects, or when autologous graft harvest is not feasible or desirable[4][5][8]. Clinical studies suggest that this approach can achieve healing rates comparable to traditional autologous iliac crest grafting but may reduce donor site morbidity[5].
