Bone marrow aspirate concentrate (BMAC) is an autologous biologic and cellular therapy utilized as an intraoperative adjuvant during hip arthroscopy. Developed and studied by the American Hip Institute, it is specifically indicated for patients with femoroacetabular impingement (FAI) and acetabular labral tears. The therapy involves harvesting bone marrow from the patient's iliac crest, which is then concentrated to isolate mesenchymal stem cells (MSCs) and growth factors. This concentrate, often combined with platelet-rich plasma (PRP) and platelet-poor plasma (PPP), is injected into the chondrolabral junction and peripheral acetabular tissue. The mechanism of action focuses on promoting tissue regeneration, modulating the inflammatory environment, and potentially halting the progression of cartilage damage and chondrolabral junction breakdown.
