Bone marrow aspirate concentrate (BMAC) is an autologous cell therapy being developed and evaluated by the Istituto Ortopedico Rizzoli for the treatment of knee osteoarthritis. It is obtained by harvesting bone marrow from the patient's iliac crest and concentrating it through centrifugation to enrich the population of mesenchymal stem cells (MSCs), hematopoietic stem cells, and various bioactive molecules, including growth factors and anti-inflammatory cytokines. When injected intra-articularly, BMAC aims to modulate the joint environment, reduce inflammation, and promote tissue repair. In the MAST-GR clinical trial (NCT04235101), it is being compared against minimally manipulated adipose tissue (MM-AT) to determine the most effective cellular product for symptomatic knee osteoarthritis, with secondary objectives focusing on disease-modifying effects and biological assessments.
