Bone marrow-derived mesenchymal stromal cells (MSCs) developed by Baylor College of Medicine are an allogeneic cell therapy being evaluated for the treatment of COVID-19-induced acute respiratory distress syndrome (ARDS). These cells are harvested from the bone marrow of healthy donors, expanded in vitro, and banked for off-the-shelf use. MSCs function through complex paracrine signaling, secreting various anti-inflammatory cytokines, chemokines, and growth factors. This immunomodulatory activity is intended to dampen the hyper-inflammatory response (cytokine storm) associated with severe COVID-19, reduce lung tissue damage, and promote the restoration of alveolar-capillary barrier function. The specific product used in Baylor's clinical trials is derived from a single donor to minimize variability and is administered via intravenous injection.
