This therapeutic regimen is a conditioning protocol for patients with multiple myeloma undergoing autologous hematopoietic stem cell transplantation (ASCT). Developed and studied by the University of Miami, the regimen combines the proteasome inhibitor bortezomib, the apoptosis-inducing agent arsenic trioxide (ATO), and the high-dose alkylating agent melphalan. The rationale behind this combination is to leverage the synergistic effects of bortezomib and arsenic trioxide in sensitizing myeloma cells to the cytotoxic effects of high-dose melphalan. Following this intensive conditioning, patients receive an infusion of their own (autologous) hematopoietic stem cells to rescue the bone marrow and restore hematopoiesis.
