Bortezomib + carboplatin + gemcitabine is a three-drug chemotherapy regimen investigated primarily for the treatment of advanced non-small cell lung cancer (NSCLC). Bortezomib is a small molecule proteasome inhibitor that disrupts the 26S proteasome, leading to cell cycle arrest and apoptosis in cancer cells. Carboplatin is a platinum-based chemotherapeutic agent that causes DNA crosslinking and subsequent apoptosis. Gemcitabine is a nucleoside analog that inhibits DNA synthesis, resulting in cytotoxicity to rapidly dividing cells. Preclinical studies have shown synergy between bortezomib and gemcitabine, with enhanced activity when combined with carboplatin. This combination has been evaluated in phase I/II clinical trials as first-line therapy for advanced NSCLC, demonstrating tolerability (mainly myelosuppression) and some evidence of efficacy[1][2][6].
