The V-Duke University-multiple myeloma regimen, often referred to as the VCR regimen, is an entirely non-intravenous combination therapy developed for the treatment of newly diagnosed multiple myeloma in patients who are not candidates for autologous stem-cell transplantation. This regimen consists of subcutaneous bortezomib, oral cyclophosphamide, and oral lenalidomide. Bortezomib is a proteasome inhibitor that induces cell cycle arrest and apoptosis; cyclophosphamide is an alkylating agent that interferes with DNA replication; and lenalidomide is an immunomodulatory drug (IMiD) that targets the cereblon E3 ubiquitin ligase complex to degrade essential transcription factors. By utilizing subcutaneous and oral routes of administration, this regimen aims to provide a more tolerable and convenient alternative for elderly patients or those with significant comorbidities, such as renal insufficiency, who may not tolerate more aggressive intravenous therapies.
