Overview

Botulinum toxin A + botulinum toxin B is a combination of two neurotoxins produced by *Clostridium botulinum*. Both toxins block acetylcholine release at presynaptic cholinergic nerve terminals, but type A and type B differ in their molecular structure and substrate specificity. Botulinum toxin A products (such as onabotulinumtoxinA, abobotulinumtoxinA, incobotulinumtoxinA) and botulinum toxin B (rimabotulinumtoxinB) are used clinically for muscle spasticity, movement disorders such as cervical dystonia, cosmetic indications (wrinkles, hyperhidrosis), and some forms of bladder dysfunction. Type A cleaves the SNAP-25 protein in the SNARE complex while type B cleaves synaptobrevin (VAMP) in the same complex, both resulting in flaccid paralysis of the target muscles[3][11]. The combination would theoretically provide effects mediated by both types, but there is no approved pharmaceutical product containing both toxins in the same formulation; their use together would be off-label or investigational.
