Overview

Botulinum toxin A + exercise is a combination therapeutic approach developed by Virginia Commonwealth University for the treatment of patellofemoral pain syndrome (PFPS). The treatment involves the intramuscular injection of 100 units of botulinum toxin A (specifically the brand Botox) into the vastus lateralis muscle of the thigh, followed by a 12-week structured exercise regimen. The botulinum toxin acts by cleaving SNAP-25, which inhibits the release of acetylcholine at the neuromuscular junction, resulting in temporary muscle weakness. In the context of PFPS, this pharmacological intervention is intended to induce a "non-surgical lateral release" by weakening the vastus lateralis, thereby reducing the lateral force on the patella. This allows for improved patellar tracking and reduced pain when combined with corrective exercises aimed at strengthening the medial musculature and improving overall knee function.
