Overview

This combination therapy is being investigated by Aswan University Hospital for the treatment of refractory erectile dysfunction in diabetic men. Botulinum toxin A (BoNT-A) is administered via intracavernosal injection to facilitate long-acting cavernosal smooth muscle relaxation by inhibiting the release of noradrenaline from adrenergic neurons. This is combined with daily oral tadalafil, a phosphodiesterase type 5 (PDE5) inhibitor that enhances the nitric oxide-cGMP pathway to further promote penile blood flow and erection maintenance. The dual approach aims to address insufficient smooth muscle relaxation and veno-occlusive disorder in patients who do not respond to standard oral or intracorporeal vasoactive treatments.
