Bremelanotide + RAAS inhibition therapy is a combination regimen being developed by Palatin Technologies for the treatment of Type 2 diabetic nephropathy (diabetic kidney disease). The investigational component, bremelanotide, is a synthetic cyclic heptapeptide and a non-selective agonist of the melanocortin receptor system, specifically targeting the MC1R and MC4R subtypes while avoiding MC2R. In the context of diabetic kidney disease, bremelanotide is administered subcutaneously twice daily to stabilize podocyte function, reduce proteinuria, and promote anti-inflammatory and tissue repair processes. This therapy is used in conjunction with standard-of-care renin-angiotensin-aldosterone system (RAAS) inhibition, which typically involves ACE inhibitors or angiotensin II receptor blockers (ARBs) to manage blood pressure and reduce renal strain. The Phase 2b BREAKOUT study (BMT-701) demonstrated that this combination significantly reduced the urine protein to creatinine ratio and stabilized the estimated glomerular filtration rate (eGFR) over six months.
