Overview

This is a combination of **brensocatib**, an oral, small molecule, reversible, selective inhibitor of dipeptidyl peptidase 1 (DPP1), and **esomeprazole**, a proton pump inhibitor. Brensocatib inhibits DPP1, blocking the activation of neutrophil serine proteases (such as neutrophil elastase, proteinase 3, and cathepsin G) in the bone marrow and reducing neutrophil-mediated inflammation[2][4][6]. It is in late-stage development for non-cystic fibrosis bronchiectasis and in trials for other neutrophil-driven diseases[6][7]. Esomeprazole suppresses gastric acid secretion by irreversibly inhibiting the gastric proton pump (H+/K+ ATPase) in parietal cells, thereby preventing the final step in acid production[8]. There is no evidence this is a marketed or investigational fixed-dose combination; in trials, esomeprazole has been used to test the effect of acid suppression on brensocatib pharmacokinetics[9].
