budesonide + simvastatin

Budesonide + simvastatin is a combination of two small molecule drugs, each with distinct mechanisms of action. Budesonide is a synthetic corticosteroid with potent anti-inflammatory properties, commonly used in the management of asthma, chronic obstructive pulmonary disease (COPD), Crohn’s disease, ulcerative colitis, and allergic rhinitis. It acts primarily by binding to the glucocorticoid receptor and modulating gene expression to suppress inflammation[1]. Simvastatin is an HMG-CoA reductase inhibitor (statin) indicated for lowering cholesterol and reducing cardiovascular risk; it works by inhibiting the rate-limiting step in cholesterol biosynthesis[4]. When combined, as studied in clinical research for asthma management, simvastatin has been shown to enhance the anti-inflammatory effects of inhaled budesonide. This enhancement occurs through increased indoleamine 2,3-dioxygenase (IDO) activity via noncanonical nuclear factor κB pathway activation and augmented IL-10 secretion from macrophages[2]. The combination has been investigated primarily for its potential synergistic effect on airway inflammation in asthma.
