Overview

This is a combination of three small molecule drugs—bumetanide, dapagliflozin, and eplerenone—each with distinct but potentially synergistic mechanisms of action primarily targeting cardiovascular and renal conditions. - **Bumetanide** is a potent loop diuretic that inhibits the sodium-potassium-chloride cotransporter (NKCC2) in the thick ascending limb of the loop of Henle, leading to increased excretion of sodium, chloride, and water. - **Dapagliflozin** is a sodium-glucose co-transporter 2 (SGLT2) inhibitor that reduces glucose reabsorption in the proximal renal tubules, promoting glycosuria and osmotic diuresis; it also has natriuretic effects. - **Eplerenone** is an aldosterone receptor antagonist (mineralocorticoid receptor antagonist), which blocks aldosterone-mediated sodium retention while sparing potassium. The combination may be considered for patients with heart failure or resistant hypertension where multiple mechanisms are targeted for volume reduction and cardiorenal protection. Studies have shown adaptive natriuretic synergy between bumetanide and dapagliflozin over time[6][9], as well as potential benefits from adding eplerenone to reduce morbidity/mortality in heart failure[5]. However, this triple therapy increases risks such as dehydration, electrolyte disturbances (notably hyperkalemia from eplerenone), hypotension, kidney dysfunction, and drug-drug interactions[3][4][8].
