This combination therapy consists of the local anesthetic bupivacaine and the neurotoxin botulinum toxin type A (Botox). It is being investigated by Yale University for the management of post-operative pain in patients undergoing mastectomy and immediate implant-based breast reconstruction. Bupivacaine provides immediate, pre-emptive analgesia by blocking voltage-gated sodium channels, while botulinum toxin provides prolonged muscle relaxation by inhibiting acetylcholine release at the neuromuscular junction. This dual mechanism targets both direct surgical pain and pain resulting from muscle spasms (particularly of the pectoralis major), potentially reducing the requirement for opioid analgesics and improving recovery outcomes.
