This triple-combination analgesic regimen consists of the local anesthetic bupivacaine, the corticosteroid dexamethasone, and the vasoconstrictor epinephrine. Studied by Lviv National Medical University, it is utilized for peripheral nerve blockade to provide prolonged postoperative analgesia, particularly in orthopedic surgery. Bupivacaine functions by blocking voltage-gated sodium channels, thereby inhibiting nerve impulse conduction. Dexamethasone acts as a glucocorticoid adjuvant to extend the duration of the sensory and motor block through anti-inflammatory effects and potential direct modulation of nerve fiber activity. Epinephrine serves as an alpha-adrenergic agonist that induces local vasoconstriction, which slows the systemic absorption of bupivacaine, further prolonging its local effect and reducing the risk of systemic toxicity.
